Individual Performance Plan

An Individual Performance Plan (IDP) is used to set goals or build a plan for focused development. Individuals should build this plan in collaboration with their supervisor.

Insert Name and Date
Goal and purpose - What will achieving this goal accomplish? How does this goal align with organizational objectives or personal purpose?
Define Success - Identify what success will look like and how it will be measured
Current Strengths - Identify strengths and skills that the individual currently possesses.
Anticipated Challenges - Identify challenges or barriers to completing your goal and how you can overcome them.
Areas of Opportunity - What particular skills will be developed with the completion of this plan?
Resources, Support and Opportunities - Identify resources, support systems and opportunities for development in relation to the plan
Scheduling Check In and Progress Update - Identify several progress check in points ahead of time where parties will come together to reflect upon plan progress.
